No Image Menu Extension handler exports two interfaces in addition to IUnknown: IShellExtInit and IContextMenu. It uses standard functions and procedures that Windows Shell calls to manage data between the extension and the Shell, like IShellExtInit::Initialize, IContextMenu::QueryContextMenu, IContextMenu::GetCommandString, IContextMenu::InvokeCommand.
